Return to School Form - issued 11/10/09
Key Points Regarding this Form
1.) Parent responsibility to notify
teacher of absence due to illness.
2.) Children who exhibit signs of
illness at school will be sent home with this new form
to be submitted upon return
3.) All children returning to school
after recovering from fever or flu like symptoms will be
assessed by the teacher and Nurse. This new form
will also be reviewed after submission
